Thomas Chittenden Health Center

Thomas Chittenden Health Center is an independent family practice serving patients of all ages, from newborns through late adulthood. Founded in 1969 as the first independent group practice in Vermont, it offers primary care from physicians, physician assistants and nurse practitioners, along with onsite lab work, digital x-rays, nutrition and diabetes education, and counseling.

Care is delivered by treatment teams, with each provider paired with a nurse or medical assistant, and visits can be in person or by telehealth. Acute appointments are available Monday through Saturday, and an on-call provider covers evenings, nights and weekends around the clock. The practice is on Oak Hill Road in Williston, Vermont, and it also hosts medical students from the University of Vermont.
