Boswell Transitional Care of Cascadia

Boswell Transitional Care of Cascadia is a rehabilitation and skilled nursing facility offering transitional care, long-term care, ventilator and tracheostomy care, and bedside dialysis. Its rehabilitation team provides physical, occupational, and speech therapy, and the respiratory team manages both short- and long-term mechanical ventilation with continuous monitoring.

Rooms are private or semi-private, and residents have an activities program and social services for families. Therapy is offered seven days a week, and the facility works with a range of respiratory diagnoses, including COPD, asthma, congestive heart failure, and traumatic brain injury. Bedside dialysis is provided in partnership with Spectrum Dialysis for residents who cannot travel to an outpatient center. The facility is at 10601 W Santa Fe Dr in Sun City.
