Aza Health

Aza Health is a private nonprofit community and migrant health center that was founded in 1977 in rural Putnam County. Its primary care is led mostly by family practice and internal medicine physicians, and it also offers dental, mental health and pharmacy services at locations across northern Florida.

Appointments are made by phone, and telehealth visits are available. The center accepts most insurance, including Medicaid and most HMOs and PPOs, and offers a sliding fee for qualified patients or special payment arrangements made at the visit. The St. Augustine office is on Whitehall Drive, across from the rear entrance of Walmart. It is open Monday through Thursday and closed Friday through Sunday.
