Lake Taylor Transitional Care Hospital

Lake Taylor is a not-for-profit transitional care hospital offering long-term acute care, ventilator liberation, physical, speech and occupational therapies, and sub-acute pediatric services. It also provides skilled and intermediate nursing care, so patients can move through a continuum of treatment after a hospital stay.

Admissions can start online or through a referral from a specialist, case manager or hospital discharge professional. The campus is on Kempsville Road in Norfolk, and visiting hours run daily from 9 am to 9 pm. Lake Taylor is governed by a board appointed by Norfolk City Council and relies on volunteers, an auxiliary and a foundation.
