Sivam Diabetes & Primary Care LLC

Sivam Diabetes & Primary Care is a primary care clinic focused on diabetes, run by nurse practitioner Dr. Mavis Ermakov with collaborating physician Dr. Andrew Baddoo. The clinic treats Type 1, Type 2, pre-diabetes and gestational diabetes, and manages related complications with individualized treatment plans. Services also include diabetes self-management education, insulin pump training, continuous glucose monitoring and medical nutrition therapy with a registered dietitian. Patients range from infants to seniors.

Walk-ins are welcome, and same-day appointments are available, with telehealth visits offered for patients who cannot come in. The office is wheelchair accessible, has parking, and is near public transportation. Daytime and evening nutrition appointments are offered. The clinic is at 1120 South Orange Avenue in Newark.
