Mindful Transitions

Mindful Transitions is a team of clinical social workers who provide counseling and social services for older adults, bringing therapy to clients in their homes or assisted living communities. The practice is led by Laura Jalbert, LCSW, and works with seniors dealing with sadness, worry, or distress tied to aging, moving to a new living situation, medical challenges, or disability. Services include individual psychotherapy, group sessions, family care counseling, and transition support for residents moving into independent or assisted living.

Sessions are usually 60 minutes, often weekly, and can be held in person or by video or phone. The team also takes on geriatric forensic consultations, involuntary commitment evaluations, and family-based support for memory loss caregiving. Its home visits cover the Atlanta area, and the office is at Johnson Ferry Road in Marietta.
