Community Hospice & Palliative Care

Community Hospice & Palliative Care is a community-based nonprofit providing hospice and palliative care for people with advanced or serious illnesses, along with grief and bereavement support, veterans programs, volunteer services and a PedsCare program for children. The organization has been licensed since 1979 and says no one is denied care because of an inability to pay.

The team answers calls 24 hours a day, seven days a week, and families can request care through the website or by phone. Care is provided at home, in long-term care and assisted living facilities, in hospitals and at its inpatient care centers. The main location is on Sunbeam Road on Jacksonville's Northside, and the organization's service area covers several counties in northeast and north-central Florida.
